Its not clear what you mean by a 'webform' in this context, but you could use either Google App Engine or Apps Script:
Google App Engine is a rich platform for building web applications and includes very good support for Cloud SQL.
Alternatively you could build your form using Google Apps Script GUI Builder and then save the data to Cloud SQL using JDBC
